If you don’t have your insurance card at your appointment, you’ll need to pay at the time of your visit or reschedule. If your insurance denies the claim, you’ll be responsible for any unpaid amount.
We practice a pathway-based model of primary care designed around measurable goals, structured follow-up, and integrated behavioral health support.
Your co-payments, coinsurance, and/or deductibles are due at the time of your visit. Usually, your insurance card will show the amount you owe, but there may be additional costs like deductibles or coinsurance. We’ll estimate your payment based on your insurance details, but you’re responsible for the final amount determined by your insurance company.
Sometimes, your insurance may not fully cover all the services provided during your visit. While we verify your coverage and accept assignment, it’s important to know that our relationship is with you, not your insurance company. Please review your insurance plan to understand what’s covered. You’ll be responsible for any services not covered by your insurance.
We offer new patient appointments based on availability.
New patient visits allow time for comprehensive evaluation and pathway development. Follow-up visits are structured based on your care plan.
Absolutely! We believe healthcare should be accessible to everyone. If you don’t have insurance, you can still see us. For self-pay patients, payment is due at the time of your visit. We accept Cash Pay, Apple Pay, Visa, MasterCard, and Discover.
Yes. We want visits to be convenient for you. While new patient visits are typically in-person, follow-up appointments may be provided through telehealth when clinically appropriate.

We ask that before you receive services that you verify that we are participating providers for your insurance company. In the event that we are not, we will file the initial claim as a courtesy for you; however, payment is due at the time of service.
We accept most major insurance plans, including Medicare, Medicaid, Blue Cross Blue Shield, United Healthcare, Cigna, Aetna, and Texas Children's. Please contact our office to verify coverage.
If you fail to cancel or reschedule your appointment 24 hours before the appointment date, your account is subject to incurring a $30 “No Show” fee.
You must provide us with your current billing address, all available telephone numbers and any other important contact information. If your address or contact information changes, it is your responsibility to contact us with the updated information as soon as possible to keep your records current.
Lab results are reviewed upon receipt and discussed during structured follow-up visits.
